Breast Ultrasonography in Young Women: Indications and Outcomes

Abstract
Purpose: Due to increasing interest of breast problems in younger women, many young women are referred for breast ultrasound (US). Therefore, we intended to investigate their symptoms, their US findings under BI-RADS category and histopathologic results.

Materials and Methods: From January to June 2006, 323 young women (less than 30 years) who underwent breast US were included in this study. Mean age was 24 year old (range: 11-29 year old). Biopsy proven malignancies (n=3) in other hospital were excluded. Their symptoms were marked as palpability, pain, nipple discharge and axillary mass and etc. Histopathologic results were compared with BI-RADS.

Results: Common symptoms were was palpability (n=159), pain (n=20), axillary mass (n=13) and nipple discharge (n=10), skin or nipple lesion (n=4). Remaining patients were asymptomatic and their visit was due to nonpalpable abnormality of breast US in other hospital (n=38), follow-up for previous biopsy (n=48), screening (n=28). US findings were classified as category 1 (n=71), category 2 (n=37), category 3 (n=166), category 4 (n=44), category 5 (n=2). There were no malignancies among category 1, 2 and 3. PPV2 of category 4 and 5 was 6.8 and 100%, respectively. Five malignancies were DCIS (n=1), invasive ductal carcinoma (n=3) and acute lymphoblastic leukemia (n=1). Among the 5 malignancies, four were symptomatic (palpable in 3, bloody nipple discharge in 1), One asymptomatic malignancy was acute lymphocytic
leukemia found in PET-CT scan.

Conclusion: The main cause of referral to breast sonography of young women is mostly palpability and followup after benign biopsy. Malignant rate of category 1,2 and 3 was 0% and positive predictive value of category 4 and 5 was 6.8 and 100%, respectively.
